The Food and Drug Administration has approved only one Quinine product, Qualaquin, and only for the treatment of malaria. But that has not stopped several firms from marketing Quinine products for the unapproved, off-label use to treat leg cramps and similar conditions -- with tragic consequences.

Since 1969, the FDA has received 665 reports of adverse events associated with Quinine use, including 93 deaths. One problem with unapproved Quinine use is that there is a very narrow range between a therapeutic dose and a toxic one. That means Quinine users can easily take a life-threatening dosage.

The most serious side effects associated with Quinine include an irregular heartbeat, thrombocytopenia (a decrease in blood platelets that can cause hemorrhage or clotting problems), blindness, hearing loss and kidney failure.

In 1994 the FDA published a regulation that prohibited the sale of Quinine to treat nocturnal leg cramps without approval. Even so, numerous drug products containing quinine sulfate continued to be marketed for off-label use.

The agency enforced more drastic measures in December 2006, ordering all firms to cease manufacturing unapproved products containing Quinine. The ban went into effect in February 2007 -- but not before thousands of unsuspecting consumers had used those dangerous products, putting their health in serious jeopardy.
